|3 - 10 mL respiratory specimen (Sputum, Transtracheal aspirate, Bronchoalveolar lavage fluid (BAL)/washings, or directly processed specimens)
|Sputum - Rinse the mouth out with water to reduce specimen contamination.
|Collect a minimum of 3 mL of clinical sample. Clinical samples should be held at 2-8 degrees C from time of collection to receipt at SHL. This can be accomplished by including an ice pack with each sample. TB Mycology specimen collection kits are available from SHL.
Sputum: Collect three specimens at least eight hours apart ensuring that at least one specimen is a first morning collection. The healthcare professional must give instructions for producing sputum. The patient’s mouth should be clean before producing a specimen. Sputum should be coughed from deep in the chest and placed in a sterile specimen container. Pooled sputum specimens are unacceptable.
|Temperature and Stability:
|Refrigerate (2-8°C); stable for 72 hours
|Leaked or respiratory samples under 1 mL. Samples with obvious food particles or solid particulates, non-respiratory specimens, unlabled or mislabeled specimens, samples processed by methods other than NALC-NaOH, decontaminated specimen sediment that is > 72 hours old, specimen sediments that have not been stored at 2-8 degrees C from time of concentration to receipt at SHL.
|MTB target DNA sequence: Not Detected, Detected, Invalid. Rifampin mutation in the rpob gene: Not Detected, Detected, Indeterminate, Not Applicable.
|Wrap specimen container in absorbent material and place each specimen in an individually sealed biohazard bag. Transport with cold packs. Specimens should be received by the testing laboratory within 24 hours of collection to prevent overgrowth of contaminants. Ship to the Coralville location.
|If TB disease is suspected, order both the "Acid Fast Bacillus Culture Panel (Mycobacterium tuberculosis Culture Panel)" and the "MTB Complex and Rifampin DNA" test for the appropriate sample type for the first two samples collected. Order the "Acid Fast Bacillus Culture Panel (Mycobacterium tuberculosis Culture Panel)" for all subsequent collections. Rapid delivery of temperature controlled specimens is essential for optimum recovery of acid fast bacilli. Positive Cepheid GeneXpert result for Mycobacterium tuberculosis complex will also identify presence or absence of rifampicin resistance gene.
