The State Hygienic Laboratory is transitioning its Rabies testing to its laboratory information system known as OpenELIS on Nov. 1. As part of this process, the Rabies Test Request Form and results report have been updated.

The new Rabies Test Request Form is available from the laboratory’s Clinical Test Request Forms webpage. Clients should choose “Rabies” from the first list and then select their clinic or hospital name from the second list. After choosing “Submit,” the client contact information is displayed on a fillable PDF. After completing all fields on the PDF, clients may print the test request form and enclose it with a specimen shipment to the State Hygienic Laboratory.

Results may be obtained more quickly through the OpenELIS Web Portal. Clients who do not have access to the portal may complete the web access form, and send it to the Hygienic Laboratory via email, fax, or mail.
