SHL updates clinical test request forms (TRFs)
Clinical Test Request Form (TRF)
If you are requesting that SHL bill an MCO for testing, please make sure to obtain the most up-to-date coverage information for the patient. As of July 1, 2019, UnitedHealthcare Community Plan will discontinue MCO coverage and Iowa Total Care will join Amerigroup as a participating MCO provider.
The updated TRFs will be available on the SHL website beginning June 27, 2019. Instructions for downloading clinical TRFs are located here.
