New Maternal Screen Test Request Form and Final Reports June 20
The new Maternal Screen Test Request Form should be used beginning June 20, to ensure that all necessary information is collected for testing by the State Hygienic Laboratory.
The new form is pre-filled with the organization’s name, address and ID. All old versions of SHL’s Maternal Screen Test Request Form should be destroyed. Instructions for obtaining the new Maternal Screen Test Request Form and examples of the new final reports are on the SHL website.
Please note that the results for Maternal Screen samples received on or after June 20 will be available through the OpenELIS Web Portal. Please go to the Test Results page or call 319-335-4358 for Web Portal assistance.
