Ordering COVID-19 Tests - Important Updates
- SHL requests that all COVID-19 samples be submitted using the Electronic Test Request Form in the SHL Web Portal. To request a Web Portal account, go to: https://www.shl.uiowa.edu/results/web_access_request.xml. If you need assistance with acquiring a web portal account, e-mail SHL-WebPortalSupport@uiowa.edu or call 319-335-4358.
- ‘Secondary Report-To’ is no longer an option. Every facility submitting samples should use their own SHL Web Portal Account.
- Every sample requires an individual electronic entry. Do not use a printed form with handwritten patient information. This causes data errors and delays results. If an error is identified after printing, you may strike out the incorrect information and handwrite the correction.
SHL is committed to providing the best turnaround time possible for COVID-19 results. Testing efforts continue to rapidly expand in Iowa and the COVID-19 Electronic Test Request Form is an essential component in expediting SHL’s results. PROPER DOCUMENTATION
Electronic Test Request Form
- Be accurate. SHL will use the exact information submitted on the form. SHL will not call the client for verification of information. Clients may call SHL to make corrections if needed.
- Provider/NPI numbers are required for each submission. Please do not use the Organization number.
Sample labels:
- Samples must be labeled with two patient identifiers that match the test request form (patient first/last name, date of birth, medical record number or patient identification number).
LABORATORY SAFETY REMINDER
Do not use staples when packaging samples. Holes in the biohazard bags may lead to spillage and exposures to live virus.
COLLECTION KIT ORDERS

COURIER UPDATE
Central Delivery Service of Iowa (CDS) is operating 7 days per week. Pick-ups for hospitals and clinics remain unchanged. Pick-ups for scheduled testing, such as with jails and long term care facilities, should be scheduled two days in advance. If you are not a current CDS client, call the SHL COVID-19 hotline for assistance at 855-374-4692.
