New SHL Web Portal Application Process
The focus of this electronic form is for clients who send SHL clinical samples (including COVID-19, newborn screening, and rabies). The SHL web portal allows for a user to download test reports and a spreadsheet of results, check on the status of a test, sign up for email notifications, and order COVID-19 testing electronically. We know that the faster you receive test results and can order COVID-19 testing, the faster you have information for your role. This new electronic application form is active now and we ask that you begin using it right away to request a new account.
It is important to note that the SHL web portal is not for patients. Patients should work through their health care provider for obtaining test results.
To apply for a SHL web portal account, clinical clients should complete this online application form. In the past, we accepted email or faxed applications, but we will only accept the online application at this time. If you currently have an account, it is not necessary to reapply. If you have forgotten your username or password, we are happy to assist if you contact us at the information below.
One exception - If you will be submitting samples for any type of environmental or safe drinking water testing, the emailed or faxed pdf/paper application form is required for the present time.
The application process for a web portal account is easy.
- Visit www.shl.uiowa.edu
- Click the green Test Results button on the left-hand side of the home page.
- Under Forms on the right side of the Test Results page:
- Clinical Samples: Click the first link, Web Portal Application Form (all clinical testing including COVID-19, newborn screening, and rabies)
- Environmental/Safe Drinking Water Samples: Click the second link, Web Portal Application Form (all environmental and safe drinking water testing)
- To discontinue paper results reports: Click the third link, Stop mailing all paper results
